The following Personal History Questionnaire's (PHQ) are required to gain access to Energy Northwest’s Columbia Generating Station nuclear facility. Please review the descriptions below and choose which PHQ fits your nuclear experience. Directions for the PHQ's are described in each document.

This is for employees who have never attained unescorted access to a US governed nuclear facility
Update
This form is for employees whose last date of favorable termination from a US governed nuclear facility is greater than 365 days but less than 3 years.
Reinstate >30 <365
Last date of favorable termination from a NRC governed nuclear facility is greater than 30 days but no more than 365 days.
Reinstate <30
Last date of favorable termination from a NRC governed nuclear facility is less than 30 days.
